Welcome to Churchwide Healthcare (CWHC) coverage through the Christian Church Health Care Benefit Trust (CCHCBT). CCHCBT has contracted with Highmark Blue Cross Blue Shield (BCBS) to act as our claims administrator.*

Pension Fund offers two traditional PPO and two high-deductible medical plan options:

Health Choice 500
Individual deductible of $500, with benefits payable at 80% thereafter. For summary of benefits & coverage click here.

Health Choice 1000
Individual deductible of $1,000, with benefits payable at 80% thereafter. For summary of benefits & coverage click here.

Health Choice 1500
Individual deductible of $1,500, with benefits payable at 80% thereafter. For summary of benefits & coverage click here.

HSA Choice 2600
A high deductible ($2,600) health plan that can be coupled with a health savings account either through Highmark’s preferred financial partner or your local bank/credit union. For summary of benefits & coverage click here.

HSA Choice 4500
A high deductible ($4,500) health plan that can be coupled with a health savings account either through Highmark’s preferred financial partner or your local bank/credit union. For summary of benefits & coverage click here.

High deductible health plans can be coupled with a health savings account either through Highmark’s preferred financial partner or your local bank/credit union. Benefits will be paid at 80% after the deductible is satisfied. If you are considering the HSA Choice 2500 or HSA Choice 4,500 plans, prescription drugs are treated the same as an office visit and you will be required to pay the full cost of the prescription until the deductible is met. Benefits continue to be paid at 80% until you reach an out- of-pocket maximum.

Preventive Services
We follow the Preventive Services Guidelines as provided by the Department of Health and Human Services. Services on this list are covered at 100% and are not subject to the deductible.
