Note: If the Covered Person chooses a name brand drug when the equivalent Generic Drug is available, the Covered Person will pay the difference in cost between the name brand and the generic drug, plus the name brand co-payment. If the Covered Person’s physician has indicated that a name brand drug be dispensed as written (DAW), the Covered Person will not be charged the difference between the name brand and the generic drug.
The formulary is a comprehensive list of preferred drugs selected on the basis of quality and efficacy by a professional committee of physicians and pharmacists. A drug formulary serves a guide for the provider community by identifying preferred drugs. It includes name brand and generic drugs. A non-formulary co-payment applied to prescription drugs not on the approved formulary list. If the Covered Person chooses to fill their long term drugs at a participating retail Pharmacy, the Covered Person will pay the mail order co-payment for a 34 day supply on the third and subsequent prescription refills.
